I recently came across an interesting??post written by Allen Power?that raised the question of whether dementia is better thought of as a psychiatric or a neurological problem. Power argues that dementia is increasingly viewed as a psychiatric illness, with symptoms of distress and analogous to mental illness. Psychiatrists are brought in as ?expert pill jockeys? to control behavioral problems with antipsychotic drugs. Power thinks this approach is wrong:
Dementia is?not?a psychiatric illness. It is a change in one?s experience of their surroundings and how they process information, based on structural neurologic changes. It is as much a psychiatric illness as would be a stroke. And people?s interpretations of the world around them may seem confused to us, but they are nothing like the symptoms of an organized psychosis.?
According to Power, the tendency to psychiatrize dementia leads us to overlook non-pharmacological interventions, which he argues have been shown to be the safest, most efficacious and most enduring ways to manage the behavioral problems associated with dementia. He concludes that it would be much better to view dementia as??a neurological disability with secondary psychological challenges,? and that psychiatry?s role should not be simply prescribing pills to control behavior ?but helping with the broader psychosocial challenges that dementia entails.
I follow and admire Power?s work as a geriatrician and one of the leading critics of the dominant drug-based approach to treating dementia. But I think that there is an important historical issue that structures the problem he raises. Without understanding and explicitly confronting this issue, efforts to change the dominant approach are not likely to have much traction.
In arguing that that dementia is a neurological rather than a psychiatric condition, Power follows the dominant modern medical approach to dementia in placing cognitive symptoms, attributed to ?structural neurological changes,? at the center and relegating emotional and psychological changes, ?to the periphery ? mere?epiphenomenal reactions to the primary cognitive damage. This is somewhat arbitrary since, for patients and family members at least, the?emotional and psychological symptoms are often as prominent and disturbing as the cognitive ones. But it follows a deep historical tendency in modern medicine to view psychiatric symptoms and mental disorders as less legitimate because they are not clearly attributable to pathological structures in the body.
This bias emerged clearly, in the United States at least, in the late nineteenth century as the development of germ theory and microbiology created a more scientific approach to medicine. Acute, infectious diseases which could be attributed to a particular pathological agent and effectively treated with a specific drug increasingly became the paradigm of modern medicine, especially as antibiotics emerged in the twentieth century. Chronic illnesses, especially psychiatric ones, seemed less legitimate, and the medical specialties that focused on them lost prestige in the era of ?the magic bullet.?
Psychiatry was further marginalized during this period by its overlap with the other medical specialty claiming expertise over the brain and mental phenomena, neurology. Though the distinction between the psychiatric and the neurological has perhaps always been somewhat arbitrary, neurologists during this period, especially in the United States, ?were generally successful in associating their specialty with cutting edge science while psychiatrists struggled under the stigma of their historic association with asylums and chronic, incurable madness.
The history of psychiatry since the late-nineteenth century can be interpreted as trying to compensate for this marginalization. Leading psychiatrists of the period, especially in Germany, sought to put their field on a scientific basis commensurate with the advance of medicine as a whole by showing that mental illness could be linked to specific brain pathologies. When this approach failed, Emil Kraepelin turned toward a quantitative assessment of clinical symptoms as a more scientific means of defining psychiatric disorders. Freud and his followers meanwhile sought to provide a scientific basis for psychiatry by turning away from the intractable problem of psychosis and developing a unified, expansive theoretical framework to explain and treat the mind. In recent decades, armed with new insights into genetics and neurochemistry and new technologies for exploring the brain, psychiatry has returned to the dream of anchoring psychiatric symptoms and disorders firmly in the brain. But apparent progress in understanding the brain only perpetuates the marginalization of psychopathology that cannot be clearly associated with something specific that is wrong with the brain. Psychiatric symptoms and clinical disorders continue to have an ambiguous status unless they are clearly associated with specific pathological processes in the brain.
While Power provides a strong critique of psychiatry?s reductionist approach to managing the ?problem behaviors? of dementia with antipsychotic drugs, in this post at least he ironically appears to endorse much of the mainstream medical concept of dementia that historically produced such reductionism. So long as a reductionist model of dementia as simply a brain disorder retains a near exclusive grip in medicine, so long as structural neurological changes are at the core and psychiatric symptoms, let alone social and cultural factors, are pushed to the periphery, medical practitioners from any specialty are much more likely to respond with a prescription pad than with the agenda for ?social and cultural change that Power would like to see.

One of the most anticipated of these features is the ability to use FaceTime over your 3G or 4G wireless connection with no wifi required. But according to a report from 9to5Mac, the new FaceTime feature may carry a heavy cost if you're an AT&T customer.
In a new developer's build of the iOS 6 software, if you try to use FaceTime without being connected to a wifi network, you'll be greeted with an error message requesting you contact AT&T. "To enable FaceTime over cellular on this account," the prompt reads, "contact AT&T at 611 or visit http://www.att.com/mywireless." The Facetime over 3G/4G functionality appears to work much like the built-in option to turn your iPhone into a wifi hotspot ? usable, but you have to pay AT&T to enable it first.
Presumably, the issue here is the sheer amount of wireless data that FaceTime gobbles up. Since AT&T iPhone users are largely grandfathered into unlimited data usage plans, allowing everyone to use the data-hungry feature could prove a major strain on the company's 3G network.

CHAMONIX, France (AP) ? It was nearly impossible to foresee the deadly avalanche on Mont Blanc, officials said Friday, as survivors of the slide that killed nine climbers described being tossed and trapped by a wave of snow that hit without a sound.
Three Britons, three Germans, two Spaniards and a Swiss climber were killed, and 14 people were injured in Thursday's accident below the summit of Western Europe's highest peak. A memorial ceremony is scheduled for Saturday afternoon in the Alpine town of Chamonix.
A French guide made the call early Thursday morning to raise the alarm about the avalanche. Early summer storms left behind heavy snow that combined with high winds to form dangerous overhanging conditions on some of the popular climbing routes. Regional authorities had warned climbers to be careful, but authorities said there was no warning of Thursday's slide.
Britain's ambassador to France, Sir Peter Ricketts, said the climbers "were doing nothing imprudent" and there were "no indications of an avalanche" ahead of time.
The area sees between four and 10 accidents per day this time of year, but rarely something so serious, said Jean-Louis Verdier, deputy Mayor of Chamonix and a mountain guide who knows the road taken by climbers.
"We didn't get any alerts of avalanche risks, but we do not always see that risk, and only when we are on (the ice sheet) we unfortunately realize that there is an avalanche danger," he said. "But yesterday, nothing alerted us, nothing indicated that that drama could happen."
The avalanche hit a group of climbers who were some 13,100 feet (4,000 meters) high on the north face of Mont Maudit, part of the Mont Blanc range. Authorities say the avalanche have been caused by a sheet of ice breaking off above the climbers or by a climber inadvertently setting a slab loose.
Survivor Daniel Rossetto, a 63-year-old guide quoted in France's Le Parisien newspaper, described being tossed and trapped by the snow and tied up "like a sausage" in his rope.
Rossetto, who was leading two Danish climbers up the mountain, said the three of them may have survived because they were on the edge of the falling slab instead of the center.
"We were on the edge of the avalanche ? that was our fortune ? while the other climbers were held under by masses of snow," he was quoted as saying. "Where we were, I didn't see the wind slab."
When the avalanche hit, he said, it was "without sound, just a gust. ... You are trapped inside, it tosses you around. With each shock, you ask yourself if it's going to get worse. It's like I was in a washing machine," he said.
He was quoted as saying he didn't expect the death toll to be so large. "I think the mountain forgives no one," he said.
The dead included the former head of the British Mountaineering Council, Roger Payne, and clients he was leading up the Trois Monts route to the 15,782-foot (4,810-meter) summit of Mont Blanc, the group said on its website.
The Mont Blanc massif is a popular area for climbers, hikers and tourists but a dangerous one, with dozens dying on it each year. Chamonix, a global epicenter for serious alpine climbing, hosted the first Winter Olympics in 1924.
Americans are experts in the art of "breaking up," and though technology allows us to dump someone much quicker, the overall message has remained constant. All break up letters (at least the detailed break up letters) have similar elements and key points: they start with an introduction mentioning "why" it has come to this point, followed by what's expected in a relationship (the word "happiness" of appears), then specific examples of the misbehavior and mistreatment, and it usually ends with the dreaded "this is for the best." Think about it: The Declaration of Independence is America's most iconic document, written by brilliant men who believed America was being crippled by taxes, exploited by England and parliament, and were not being treated as well as they felt they deserved. These factors and more led our founders to literally pen a detailed letter (the "Declaration") to King George and England, and possibly the greatest breakup letter in history. To prove my point, I'm going to provide excerpts from the "Declaration," followed by how it translates into actual breakup letter terminology.
The Declaration: "When, in the course of human events, it becomes necessary for one people to dissolve the political bands which have connected them with another..."
Translation: There comes a point in every relationship when two people can't work it out, and have to break up and move on, regardless of all the good times.
The Declaration: "The history of the present King of Great Britain is a history of repeated injuries and usurpations, all having in direct object the establishment of an absolute tyranny over these states... He has refused his assent to laws, the most wholesome and necessary for the public good."
Translation: "For the longest time you've neglected me, disrespected me, take me for granted and try to control every aspect of my life. You no longer tell me you love me, that I'm beautiful or care about my happiness."

So, the next time you want to breakup with someone and don't know what to words to write, email, text or tweet; all you have to do is find an online text of the "Declaration of Independence," make some minor changes, such as "King George" to "A@#hole" or "B*#ch," and you'll have a breakup letter that our founding fathers would be very proud of.

TWENTYNINE PALMS?Marines with 9th Communication Battalion set up supporting communications and data transmission services for 1st Marine Expeditionary Brigade during Javelin Thrust 2012 in Twentynine Palms.
The Marines arrived June 18 to set up the brigade?s communications. The exercise serves a part of a three exercise continuum aimed to validate the readiness of 1st MEB as a global crisis response force.
The battalion embarked from Camp Pendleton with command and control center tents, gear and equipment and more than 46 tactical vehicles designed to operate a command and control center.
Javelin Thrust differs than typical training exercises aboard Camp Pendleton, where battalion is based, by supporting more than 400 Marines and sailors of the 1st MEB command element.

Over the past ten years, the battalion has typically deployed to environments?such as Iraq an Afghanistan?where communications networks were already in place. Javelin Thrust provided the battalion to establish its command and support system from the ground up.
?This is a culminating event,? said Maj. Fernando Jimenez, the unit?s Javelin Thrust detachment officer in charge. ?This is what it?s going to take to support a MEB should it be called upon. Our ability to command and control is being validated, and we are succeeding.?
The battalion provided Marines with various occupational specialties to equip 1st MEB with the assets and knowledge they?ll need to maintain communication during the exercise. Sgt. Steven S. Pulliam, a motor transport mechanic, and his Marines took responsibility of providing the battalion day-to-day resources.
?We support the MEB with water, fuel, food and transportation,? said Pulliam, who serves with the battalion. ?One of the big things we do here is keep the generators running and make sure the water bulls [water tanks] are clean and full of water for everybody.?
Keeping the equipment cool in the Mojave Desert training area is a challenge for the unit as the temperature peaked higher than 100 degrees.
?I?m in charge of making sure that the generators and air conditioners are running,? said Cpl. Francisco R. Cruz, an AC. ?We supply the power and necessary temperature to keep the gear cool so it runs in its proper function.?
Communication between the brigade?s command operations center and the ground units is a critical function to conduct a large-scale exercise. Maintaining a communication system is vital to keep operations fluid.
?I?m here supporting the technicians we have from the other satellite systems for communication,? said Lance Cpl. Michael P. Raymond, a multi-channel communications equipment. ?We?re the ones that make sure communication goes through smoothly and any data or commands that are given are sent through.?
Sgt. Arthur I. Baronov, a technical controller, is responsible for installing, maintaining, operating and troubleshooting tactical communication networks for the Marine Corps and coalition forces in a deployed environment. Technical controllers typically work with the majority of the communication equipment as subject matter experts for the other Marines in the unit.
?We act as the central hub for coordinating the troubleshooting and establishment of a fully functional communications network using tactical gear,? Baronov said. ?The biggest part of our role is to monitor our network and make sure we have reliable communication.? The battalion also provides communication from the sky via satellite.
The Marines training on the ground communicate with other units through field radios. They rely on the radio operators to monitor and maintain connectivity with the command element. Sgt. Patricia L. Reynolds, a field radio, said her Marines had communication support ready for the Marines training in a matter of days upon arrival.
Every phone call, e-mail, field radio command and satellite transmission goes through the battalion?s equipment. The exercise hinges on the battalion?s ability to do their job and keep the momentum.
?It?s all about communications to effectively command and control,? Jimenez said. ?We?re doing our part. The Marines have done a fantastic and phenomenal job. From the day they hit the ground running, they?ve not stopped.?

A team of astronomers using NASA's Hubble Space Telescope is reporting the discovery of another moon orbiting the icy dwarf planet Pluto.
The moon is estimated to be irregular in shape and 6 to 15 miles across. It is in a 58,000-mile-diameter circular orbit around Pluto that is assumed to be co-planar with the other satellites in the system.
"The moons form a series of neatly nested orbits, a bit like Russian dolls," said team lead Mark Showalter of the SETI Institute in Mountain View, Calif.
The discovery increases the number of known moons orbiting Pluto to five.
The Pluto team is intrigued that such a small planet can have such a complex collection of satellites. The new discovery provides additional clues for unraveling how the Pluto system formed and evolved. The favored theory is that all the moons are relics of a collision between Pluto and another large Kuiper belt object billions of years ago.
The new detection will help scientists navigate NASA's New Horizons spacecraft through the Pluto system in 2015, when it makes an historic and long-awaited high-speed flyby of the distant world.
The team is using Hubble's powerful vision to scour the Pluto system to uncover potential hazards to the New Horizons spacecraft. Moving past the dwarf planet at a speed of 30,000 miles per hour, New Horizons could be destroyed in a collision with even a BB-shot-size piece of orbital debris.
"The discovery of so many small moons indirectly tells us that there must be lots of small particles lurking unseen in the Pluto system," said Harold Weaver of the Johns Hopkins University Applied Physics Laboratory in Laurel, Md.
"The inventory of the Pluto system we're taking now with Hubble will help the New Horizons team design a safer trajectory for the spacecraft," added Alan Stern of the Southwest Research Institute in Boulder, Colo., the mission's principal investigator.
Pluto's largest moon, Charon, was discovered in 1978 in observations made at the United States Naval Observatory in Washington, D.C. Hubble observations in 2006 uncovered two additional small moons, Nix and Hydra. In 2011 another moon, P4, was found in Hubble data.
Provisionally designated S/2012 (134340) 1, the latest moon was detected in nine separate sets of images taken by Hubble's Wide Field Camera 3 on June 26, 27, 29, and July 7 and 9.
In the years following the New Horizons Pluto flyby, astronomers plan to use the infrared vision of Hubble's planned successor, NASA's James Webb Space Telescope, for follow-up observations. The Webb telescope will be able to measure the surface chemistry of Pluto, its moons, and many other bodies that lie in the distant Kuiper Belt along with Pluto.

A team of astronomers using NASA's Hubble Space Telescope is reporting the discovery of another moon orbiting the icy dwarf planet Pluto.
The moon is estimated to be irregular in shape and 6 to 15 miles across. It is in a 58,000-mile-diameter circular orbit around Pluto that is assumed to be co-planar with the other satellites in the system.
"The moons form a series of neatly nested orbits, a bit like Russian dolls," said team lead Mark Showalter of the SETI Institute in Mountain View, Calif.
The discovery increases the number of known moons orbiting Pluto to five.
The Pluto team is intrigued that such a small planet can have such a complex collection of satellites. The new discovery provides additional clues for unraveling how the Pluto system formed and evolved. The favored theory is that all the moons are relics of a collision between Pluto and another large Kuiper belt object billions of years ago.
The new detection will help scientists navigate NASA's New Horizons spacecraft through the Pluto system in 2015, when it makes an historic and long-awaited high-speed flyby of the distant world.
The team is using Hubble's powerful vision to scour the Pluto system to uncover potential hazards to the New Horizons spacecraft. Moving past the dwarf planet at a speed of 30,000 miles per hour, New Horizons could be destroyed in a collision with even a BB-shot-size piece of orbital debris.
"The discovery of so many small moons indirectly tells us that there must be lots of small particles lurking unseen in the Pluto system," said Harold Weaver of the Johns Hopkins University Applied Physics Laboratory in Laurel, Md.
"The inventory of the Pluto system we're taking now with Hubble will help the New Horizons team design a safer trajectory for the spacecraft," added Alan Stern of the Southwest Research Institute in Boulder, Colo., the mission's principal investigator.
Pluto's largest moon, Charon, was discovered in 1978 in observations made at the United States Naval Observatory in Washington, D.C. Hubble observations in 2006 uncovered two additional small moons, Nix and Hydra. In 2011 another moon, P4, was found in Hubble data.
Provisionally designated S/2012 (134340) 1, the latest moon was detected in nine separate sets of images taken by Hubble's Wide Field Camera 3 on June 26, 27, 29, and July 7 and 9.
In the years following the New Horizons Pluto flyby, astronomers plan to use the infrared vision of Hubble's planned successor, NASA's James Webb Space Telescope, for follow-up observations. The Webb telescope will be able to measure the surface chemistry of Pluto, its moons, and many other bodies that lie in the distant Kuiper Belt along with Pluto.

WOW: Hi Randy, thank you for taking the time out to tell us about your event today. What does the self-publishing and book marketing workshop entail?
Randy: On the first day, the focus will be for those who are writing a book and want to understand HOW the book publishing industry works and the steps required to self-publish your book SUCCESSFULLY! So many authors ?dive in? to self-publishing unprepared without understanding the specific steps necessary to make a book successful. The result is poorly edited and poorly packaged books--ones that look, read, and scream out, ?I?m self-published!? In almost every case, these books don?t achieve the success the author intended; and often it?s not because the writing and ideas presented aren?t good, but rather that the author missed the BASIC, but CRITICAL, elements of the publishing process.
On the second day, we will cover book marketing. Now that you have a book that READS and LOOKS great, it?s time to learn how to market it effectively! Randy and Jan will share with you steps--the necessary steps that every author should take in order to spread the word about a book--as well as share specific sales, marketing, and public relations ideas for self-published authors, including the specific marketing steps for some of their most successful titles!
WOW: That's a jam packed two-day workshop! Who should attend this event?

This is the most fun and educational portion of the day!
Day 2 - Sunday, July 29, From Press to Success! How to Market Your Book Effectively in Today's Publishing World. 1) Author Platform- We will share with you the definition and metrics of a successful author platform and provide specific steps on how you can build yours! This is by far the most important step you can take in making your book successful. 2) Getting Your Book Noticed- How to get radio interviews, book signings, and other media outlets to cover your book. Also, we will share steps you can take to get your book into book stores and other ways that will increase the awareness of you and your message. 3) Press Release and Online Marketing- The basics of a good press release and how to write one, plus ideas for how to promote your book online and launch a ?bestseller? campaign. 4) Master Your Spoken Word- How to talk about your book effectively in two minutes or less (it's surprising how many first time authors aren?t able to do this!). This is SO important when it comes to doing radio and press interviews, speaking engagements, and other personal encounters, where the goal is to move people to buy your book immediately! 5) The Author Business Model- specific strategies and examples on how you can increase revenues in addition to selling books (workshops, online courses, related products, etc.), and how doing these things will in turn increase the sales of your book.

Randy: We receive over three thousand book submissions a year. UT Southwestern Medical Center eyes operating new facility
DALLAS July 9, 2012 UT Southwestern Medical Center, seeking to broaden an already robust array of radiation oncology treatments it offers to patients, potentially could play a key role in operating a state-of-the-art proton therapy center for North Texas being planned and funded by San Diego-based Advanced Particle Therapy (APT).
The plan, announced today, is subject to the approval of the UT System Board of Regents. Under the terms of a letter of intent, the proposed 100,000-square-foot center would be operated by UT Southwestern physicians and staff, but built and developed by APT. It would be the second such facility in the state and would feature at least four treatment vaults, as well as laboratory space for researchers.
As one of the most advanced radiation treatment technologies available, proton therapy offers a more precise and aggressive approach to destroying cancerous tumors than conventional X-ray radiation. Proton therapy involves the use of a controlled beam of protons that can be deposited, because of their heavier mass, on a specific target with less collateral damage.
"This is an exciting development in our ability to offer patients in the region access to the best possible treatment options and provide a leadership role in how best to study and apply this technology," said Bruce Meyer, M.D., Executive Vice President for Health System Affairs at UT Southwestern.
There are many types of particles that can be used in radiation treatment, although only two electrons and protons are currently in wide clinical use.
Pediatric patients and adults with brain, prostate, lung, and head and neck tumors may be among the first to benefit from the new technology.
"The precision of the proton beam allows for unprecedented focus and intensity in especially hard-to-reach places that tend to characterize head and neck cancers and in pediatric cancers, where access can be anatomically much tighter," said Hak Choy, M.D., Chairman of Radiation Oncology at UT Southwestern.
The heart of the new proton facility would be a particle accelerator capable of speeding up protons to a super-fast velocity roughly 112,000 miles per second or 60 percent the speed of light. Two types of machines either a superconducting cyclotron or a synchrotron can be used for this purpose. UT Southwestern officials would evaluate and select an accelerator early in the project's development.
APT has entered into an agreement to purchase land for the facility near the UT Southwestern campus, accessible to both physicians and patients alike.
Nationally, there are nine proton centers in operation and eight in development, according to the National Association for Proton Therapy. APT has developed similar centers in San Diego, teaming with Scripps Health and Scripps Clinic Medical Group; in Baltimore with the University of Maryland School of Medicine; and in Atlanta with Emory University Healthcare.

WILTSHIRE golfer David Howell is hoping his tied fourth finish at the French Open will be the catalyst for a return to his best.
The Broome Manor professional shared the overnight lead with Dane Anders Hansen going into the final round in France, and despite losing the lead thanks to a bad start to his final 18 holes, he battled back to earn his first top 10 finish in nearly a year.
Howell looked comfortable throughout his four rounds in France, and although he ended three shots behind eventual winner Marcel Siem, the former Ryder Cup player is hopeful the impressive finish will help him climb the world rankings from his current position of 350.
?All in all I am delighted,? he said.
?Maybe I didn?t finish it off and I am disappointed I wasn?t able to go on and win, but there are so many positives to take from the week here and overall I played some pretty good stuff.
?I am very happy with the way I hit the ball, and maybe it could have been better, but I am proud, especially as I have never made the cut here before.
?It is nearly a year now since I last finished in the top 10, and it has been hard, but hopefully now I can do it on a more regular basis.
?It should move me up the rankings a fair bit and the prize money is good too, so hopefully I can keep it up and keep getting better.?
Howell?s final round at Le Golf National started in the worst possible way with bogeys at the first and third holes, but he was pleased with the way he battled back to earn himself a top five finish.
?I snatched at the tee shot on the third and that is really not what I wanted to do at all, but I came back into it at six and that was pleasing.
?I had numerous chances that could have gone down but they didn?t, and I was too far back by the time I got to the end of my round.
?The last four holes are hard, and you can only really hold on on them which is what I did.?
Howell went into the event knowing a top five finish would give him a chance of earning the one spot on offer for The Open Championship in two weeks time, but despite achieving his goal he missed out because Siem had also not already qualified for the tournament at Royal Lytham & St Annes.
?The first thing I asked when I came off the course was whether Marcel had already qualified, and I was told he hadn?t,? Howell said.
?It would have been great to play at The Open, but there is another chance to get there in Scotland next week and that is my target now.?
